AGREEMENT BY APPLICANT
  1. I will make an appointment at ARF by calling (631) 537-0400 x 211 or at the assigned vet one week BEFORE I intend to trap. (Surgeries are performed at ARF on Tuesdays, Wednesdays and Thursdays.)
  2. If using ARF, I will bring the cat(s) to the ARF Medical Wing between 9:00 and 9:45am on the scheduled day. If I am unsuccessful at trapping the cat(s), I will notify ARF or the assigned vet immediately.
  3. I will pick up the cat(s) the next day at the ARF Medical Wing from 9:00 - 9:45am or at the assigned vet.
  4. I understand that I am responsible for returning the cat(s) to their previous location and I will provide them with food, water and shelter.
